Job Description:

  • assesses and evaluates members' needs in a telephonic environment
  • achieve and maintain wellness state by guiding members/families toward and facilitating interaction with resources appropriate for care
  • work assignments are varied and frequently require interpretation and independent determination of the appropriate courses of action
  • report to the Manager, Care Management
  • support the Transition of Care (TOC) program
  • assist with HEDIS completion follow up visits

Requirements:

  • Must live in the state of South Carolina
  • Must have an active license in South Carolina in one of these; either a Licensed Professional Counselor (LPC), Licensed Independent Social Worker - Clinical Practice (LISW - CP), Licensed Masters Social Worker (LMSW), Licensed Baccalaureate Social Work (LBSW), Registered Nurse (RN) OR Licensed Practical Nurse (LPN)
  • 1 or more years of related work experience
  • Able to travel 25%
