Overview
We are urgently seeking compassionate and dedicated Direct Support Professionals (DSPs) to join our team and make a meaningful difference in the lives of individuals with developmental disabilities. In this vital role, you will provide personalized support, foster independence, and promote community engagement for those you serve. Bilingual skills in Spanish and proficiency in American Sign Language (ASL) are highly preferred to ensure effective communication and inclusive care.

Duties
Offer educational, advocacy, and support resources to child/youth and caregiver/family
Caregiver/Family Supports and Services can be provided in the home of the youth or a community setting.
Help teach the caregiver/family how to access resources independently and ensure appropriate services are received by their child
Address issues the caregiver/family is concerned about (as appropriate) in relation to supporting their child
Provide instruction to allow child/family to have a fully understanding of the child’s condition or illness
Educate and train the caregiver/family unit on resource availability so that they can better support and advocate for the needs of the child and appropriately access needed services.
Development and updating of service plans, including obtaining medical consenter signatures.
Obtaining consents and enrollment documents for newly enrolled children/youth.
Attend monthly waiver service provider meetings to stay current on practices and protocols.
Report incident(s) to supervisor upon discovery.
Maintain open communication with other service providers (e.g., Care Manager) to report on child/youth’s progress and identify any potential barriers and referrals.
Submit progress notes, service plans, expense logs, and service provider logs within set forth-required timeframes. Progress notes are due within 3 days of service provision so that approval from supervisor is completed by the 5th business day following service provision.
