Program Specifics:

§ The Case Manager will provide information, assistance, and navigation relation activities in support of veterans and their families specific to Freedom Springs population; including resource referrals, financial assistance, housing navigation, follow-ups, case management, and advocacy to access and receive needed services

§ This position is housed at Freedom Springs and works with veterans that have transitioned from homelessness into housing on site. These clients will assess at a scale from low to moderate/high risk. Knowledge of working with homeless individuals, HMIS, VISPDATs, de-escalation skills, and rapport building with guarded individuals is highly desired.

Specific Duties:

§ Clients:

o Provide one-on-one, telephone, and email assistance to access barriers for clients o Assist clients in applying for resources needed and linking them to wrap around services that Mt Carmel Veteran Service Center and other community partners can provide o Conduct follow-ups with clients to ensure services are assessed and identified as well as assist with additional services and track outcomes for clients.

o Communicate with community partners on behalf of the client while advocating their needs o Create a plan of action to access clients’ barriers and set goals for moving forward o Remain diligent with clients until cases are resolved o Maintain client records and input into database. Case notes and follow-up calls must be made within 2 business days

§ Community:

o Knowledge of an array of resources and continued awareness to remain current on resource availability, resource accessibility guidelines to include but not limited to: housing assistance, community resources, veterans’ benefits, family programs, legal, childcare, financial resources, and wrap around services provided at Mt.

Carmel Veterans Service

Center o Attend monthly meetings and/or outreach events in the community as instructed by the Director of Military, Veteran, and Family Services o Attend resource fairs as instructed by Director of Military, Veteran, and Family Services o Research and explore community partner options to assist clients

§ General Support:

o Ability to develop and maintain working, professional relationships with Mt. Carmel staff, partners, and referral agencies o Ability to empathetically work with individuals of all ages and diverse backgrounds o Ensure a high level of customer service to clients o Works some nontraditional hours, including evenings and weekends as necessary and required for this position

Pay: $25.00 per hour
