[Remote] Humana Beginnings Care Coach

Note: The job is a remote job and is open to candidates in USA. Humana is a leading U.S. healthcare company dedicated to helping individuals achieve optimal health. The Care Coach evaluates and supports members' wellness needs through resource coordination, monitoring care progress, and facilitating interactions with healthcare services, primarily for prenatal and postnatal members.

Responsibilities

• Evaluate member's needs and requirements to achieve and/or maintain optimal wellness state by guiding members/families toward and facilitate interaction with resources appropriate for the care and wellbeing of members
• Work directly with members enrolled in the Humana Beginnings program and may require in person visits with members
• Employ a variety of strategies and techniques to support a member's optimal wellness state by coordinating services & resources
• Identify and resolve barriers that hinder effective care
• Ensure patient is progressing towards desired outcomes by monitoring patient care through use of assessment, data, conversations with member, and active care planning for prenatal and postnatal members

Skills

• Must be a South Carolina Resident
• LPN, RN or bachelor's degree in health and human services field
• 1 or more years of maternity experience
• Knowledge of Microsoft Office Word and Excel
• Exceptional verbal/written communication and interpersonal skills
• Must have a separate room with a locked door that can be used as a home office to ensure continuous privacy while you work
• Mother Baby experience
• Prior experience with Medicare & Medicaid recipients
• Previous Care management experience
• Previous experience with electronic case note documentation and with documenting in multiple computer applications/systems
• Experience with health promotion, coaching and wellness
• Knowledge of community health and social service agencies and additional community resources
