Overview
We are seeking a dedicated and knowledgeable Staff Pharmacist to join our healthcare team. The ideal candidate will possess a strong background in clinical pharmacy, with experience in acute care settings, long-term care (LTC) pharmacy, and a commitment to providing exceptional patient service. As a Staff Pharmacist, you will play a crucial role in patient assessment, medication management, and ensuring the safe and effective use of medications.

Responsibilities

  • Provide comprehensive pharmaceutical care to patients, including medication therapy management and patient counseling.
  • Conduct patient assessments to evaluate medication needs and outcomes.
  • Collaborate with healthcare professionals to develop and implement individualized treatment plans.
  • Compounding medications as required, following aseptic techniques and safety protocols.
  • Administer immunizations and manage medication administration processes.
  • Maintain accurate inventory management practices to ensure the availability of medications.
  • Utilize pharmacy software and electronic health record (EHR) systems for documentation and patient care activities.
  • Monitor patient responses to medications, particularly in critical care and pediatrics settings.
  • Ensure compliance with HIPAA regulations and maintain confidentiality of patient information.
  • Participate in pain management strategies and provide education on medication use for acute care patients.

Requirements

  • Doctor of Pharmacy (Pharm.D.) degree from an accredited institution is required.
  • Completion of a pharmacy residency or equivalent clinical pharmacy experience is preferred.
  • Experience in LTC pharmacy or hospital settings is highly desirable.
  • Strong knowledge of medical terminology, physiology, anatomy, and pharmacology.
  • Proficiency in IV insertion and IV infusion techniques is an asset.
  • Familiarity with EMR systems for efficient patient documentation is essential.
  • Excellent communication skills with a focus on patient service and care coordination.
  • Ability to work collaboratively within a multidisciplinary team environment.

Pay: $50.00 - $65.00 per hour
